What do the Facility Fees cover?
Great Basin Surgical Center fees cover the use of this facility only.
They do not include laboratory, pathology, physician or
anesthesiologist fees. You will be billed separately for these
services. Please be aware that you will receive several
individual bills.
How is my Insurance billed?
This facility will bill your primary and secondary insurance
carriers or governmental agency directly for the facility
fees. Be sure to bring your most current insurance,
Medicare or public assistance card with you on the day
of your surgery. If you have more than one insurance
carrier, we will also need accurate secondary billing
information.
Please be aware of your insurance plans admission policies.
You or your physician may have to adhere
to certain requirements in order to ensure maximum
reimbursement. Failure to obtain pre-authorization,
physician referral or a second opinion may greatly
reduce or eliminate your benefits.
What Payments am I required to make?
Be prepared to pay any co-pay or deductible amounts
on the day of your surgery. Patients who do not have
insurance coverage or those having cosmetic surgery
are required to pay their charges in advance.
We realize that at times you may require special financial
arrangements. In these instances, please call our business
office prior to your surgery to discuss alternative methods
of payment.
What Methods of Payment do you accept?
Great Basin Surgical Center accepts cash, cashiers checks, credit cards
and personal checks with a valid I.D.
Is financing with a payment plan available?
We at Great Basin Surgical Center would like to introduce to you a method of financing we offer in our office. You apply for the financing in our office - a very quick and easy process. Acceptance rates are high and the financing is for any necessary treatment. Similar to a major credit card, minimum payments are made each month or you may pay the balance off early without penalty. You determine if you make low payments over a long period of time or several larger payments. This is a revolving payment plan so with every payment you make, your balance is reduced and your next payment becomes smaller.
As healthcare costs continue to rise, we have to make decisions which enable us to provide you excellent care while also maintaining the cost to you. To concentrate solely on healthcare needs, we need to get out of the banking business. As per our policy, all unpaid balances on accounts are to be paid no more than 90 days after date of service. If you wish to be billed for your healthcare service, our staff will be happy to help you establish a convenient payment schedule.
